Gluing Techniques for Corneal Perforations

Summary: Corneal perforations are ophthalmic emergencies that can result in significant ocular morbidity, including decreased vision, blindness, and loss of the eye. Prompt closure of the perforation is essential to preserve corneal integrity and prevent secondary complications such as endophthalmitis, anterior synechiae, and secondary glaucoma. Available treatments range from temporizing measures, such as tissue adhesive application with a bandage contact lens, to definitive surgical repair, including corneal transplantation. Tissue adhesives, such as cyanoacrylate derivatives, have become well-established tools in the management of small corneal perforations (≤3 mm) and areas of impending perforation.

This article will: (1) review the currently available tissue adhesives for the management of corneal perforations, including their respective indications, advantages, and limitations; (2) identify the clinical scenarios in which tissue adhesive application may be considered as a primary or temporizing treatment option; and (3) provide step-by-step illustrated instructions demonstrating various glue application techniques for the management of corneal perforations.
